Facility and Resident Monitoring Service

 

 

SeniorWatch^ä is a monitoring service for families who have responsibility for a loved one residing in a nursing home or assisted living facility.  The SeniorWatch^ä   program is provided by SeniorCare Consulting^ä and includes the following services on a monthly basis;

 

Facility Check including:

-         Changes in facility survey information

-         Changes in key personnel

-         Changes in financial/operational policies, charges, etc

-         Changes in facility vendors, menus, activities, etc

-         Changes in services/programs offered

-         Changes in company ownership

 

Resident Check including:

-         Visual check; report appearance, personal hygiene, general overall condition

-         Visit with resident; report to family what resident was doing at time of visit

-         Ask resident about their concerns (favorable/unfavorable) and report comments to family

-         Consultation with nursing personnel and chart review if available; report changes in condition, recent falls, mental health and medical concerns, weight issues, etc

-         Pharmaceutical check; report all medications currently taken by resident

 

Summary Report detailing findings

-         Report will be emailed to family with hard copy delivered by mail
